Hyaluronidase or Hyalase is an enzyme which breaks down hyaluronic acid fillers. One of the great advantages of the hyaluronic acid fillers is that they can be dissolved by the injection of hyaluronidase in a minor or major complication. Minor complications include residual palpable or visible material, blanching, nodules, and hypersensitivity reactions.

Major complications include migration of the filler, granuloma formation, and infection. Vascular compromise is a major complication and requires emergency Hyalase treatment. It can be difficult to deal with a filler that hasn’t turned out the way you desire. Especially when you expected natural results. Filler treatments shouldn’t turn a small insecurity into a larger insecurity by having them performed in a non-professional manner.

How do I know if I
need my filler dissolved?

Product migration: Hyaluronic acid dermal filler is soft and although placed carefully can move through soft tissue to sit in areas of least resistance. This is why we warn clients not to exercise or sleep on the side of the faces after treatments such as cheek or jaw filler for at least 48 hours. Alternatively a client might suffer swelling which similar to the above compressions will move the filler away from the original position.

Poor filler placement: varying dermal fillers have differing texture ranging from firm to soft. Combined with certain fillers marketed for differing areas of the face; for example lips vs jaw – there is a risk that harder filler placed in the soft lips will be more noticeable. Additionally poor technique and lack of massaging will leave lumps of filler rather than smooth integrated filler.

Emergency: rarely a practitioner will block a blood vessel when injecting filler into the soft tissue. This will lead to a vascular occlusion. The effect maybe instant or if partially blocked then over time. The effects are serious in that reduced blood flow will starve tissues of nutrients and oxygen leading to necrosis.

What is involved

A patch test is required to check any allergic reaction to the Hyaluronidase (the active agent injected in the area of dermal filler (if hyaluronic acid based). If there is no reaction then those areas of dermal filler which are no longer wanted will be injected with the Hyaluronidase.

Depending on the amount, layout and age of the filler being targeted the results have normally started to be visible after a day once initial swelling has gone but it may take two weeks for the majority to disappear. It is not uncommon for a second visit to be necessary for problem lumps.Dermal fillers are aesthetic gel-based treatment injected into the skin. You must be over 18 to receive a dermal filler treatment and therefore we would not expect to treat anyone for filler dissolving who is under 18. Dermal fillers should not be used in or near areas where there is or has been skin disease, inflammation or skin related problems. Hyaluronidase has not been tested in pregnant or breast-feeding women, individuals with medical conditions that lower the body’s immune response or individuals undergoing therapy that is immunosuppressive may be more prone to infection by a skin injection procedure. We do not recommend having the treatment if pregnant but there is no scientific evidence to support it damaging your foetus.

At your initial consultation, we take a history and examine your area of concern and create a plan to dissolve your filler as appropriate. Sometimes several sessions of dissolving are required to get rid of all filler and this can only be carried out with HA fillers. Other permanent or semi-permanent material cannot be dissolved with hyalase.

About 1 in 7000 patients are allergic to hyalase. We always do a ‘patch test’ before treatment to be sure that you are not allergic. This involves introducing a very small amount of hyalase under the skin of the forearm and observing the area for 30 minutes. Adding filler after hyalase treatment is avoided generally for about 2 weeks.

This will really depend on the individual. The injections themselves feel like a slight pinch, and you may experience slight pressure or a ‘stinging’ sensation as the product is being injected. Depending on the area, your cosmetic injector may apply numbing cream to the area to be treated.

Side effects are minimal and typically relate to the local injection. Soreness, swelling or mild bruising, may occur on or around the injection site. In rare instances an allergic reaction can occur, which is why we always patch test first.

You should start to see a visible improvement immediately. Minor swelling and bruising may take 1-2 weeks to completely resolve.
